Nurse (Case Manager)

Nurse (Case Manager) for the Colorado River Service Unit in Parker, AZ. Position will provide professional nursing case management, utilization management, and discharge planning services by independently planning and executing inpatient case management activities, coordinating care for patients with complex acute and chronic health conditions, and serving as a clinical resource to other staff. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
