Job description

 POSITION: Licensed Practical Nurse (LPN)– Mental Health Care – Outpatient Clinic

$5,000 First year bonus!

LPN in this role is responsible for managing the walk-in medication clinic and nurse line, managing the injection clinics and performing additional nursing duties as required and assigned.

LPN DUTIES & RESPONSIBILITIES:

Manage Walk-in Medication Clinic and Nurse Line

  1. Walk-in Medication Clinic
    • Complete vitals for Walk-in Clinic Consumers that report they are out of medications or will not have enough medication to get to their next appointment, or consumers who report feeling poorly physically.
    • Document vitals (blood pressure, weight, and pulse) clearly for prescribers and flag any vitals outside normal limits.
    • Assist Registered Nurse with calling in orders to FCS pharmacy or other outside pharmacy.
    • Assist RN with speaking to prescribers face to face for orders and follow through with prescriber’s orders, reporting to RN the plan of action. (LPN’s cannot receive verbal orders over the telephone)
    • Assist Consumers to follow through with plan, i.e. call in prescription orders, set up next appointment.
    • Complete documentation
      • Document Consumer needs
      • Document that plan of action was executed and completed
  2. Nurse Line
    • Retrieve messages hourly and respond same day or if the message was left after 4pm return the call the next day no later than 12:00pm 

Additional LPN duties Manage Injection Clinics

  1. Complete vitals (blood pressure, weight, and pulse)
  2. Obtain and review prescriber injection orders
  3. Verify dosage changes or prescriber orders
  4. Administer injections as ordered by prescriber
  5. Re-assess vitals and re-assess Consumer 10 minutes after the injections.
  6. Schedule next injection appointment with Consumer to ensure proper injection scheduling.
  7. Injections that are related to prescriber appointments take priority over walk-in medication clinic consumers --
  8. Keep track of injection consumers with a calendar.
    • Schedule injections in Anasazi using correct phone numbers so call backs can be completed by automated system.
  9. Review and update medication record.
  10. Ensure adequate supplies for Nurses office, i.e. needles, Band-Aids, medications, and other necessary supplies and order through pharmacy.
  11. Obtain Medication Education Material for Consumers.
  12. Monitor time frames carefully and educate Consumers with maintaining scheduled injections and prescribed medications.
  13. As assigned, provide medication education to Adult Consumers both individually and in groups i.e. Psycho-social Rehabilitation.
  14. Communicate daily with other medical team members to ensure coverage and wrap around care for all Consumers. Nursing Medical Team will meet each morning for 10 minutes in order to discuss tasks and plan of coverage for each day.
  15. Complete prior authorizations, fax to appropriate insurance provider and document approvals. Bring needed medication changes or trials to RN for staffing with prescribers.
  16. Perform Phlebotomy when ordered by prescribers.
    • Spin down samples and refrigerate.
    • Document procedure in patient chart.
    • Ensure samples are taken downstairs for pick up every day by 5pm.
  17.   Check in with RN Supervisor prior to leaving for lunch or end of day, in order to ensure all tasks and Consumers are properly cared for.
  18. This position requires flexibility to work at multiple clinic locations based on operational and staffing needs. The selected candidate will be expected to float between assigned locations (Graves, Gatesway, and Lapidus Center) as needed to ensure adequate patient carer and clinic coverage. Reliable transportation is required. 

LPN QUALIFICATIONS: 

    • Requires completion of Licensed practical nursing Program and current license as an LPN in the State of Oklahoma.
    • Previous experience working with clients with mental illness and or substance abuse is preferred.
    • Requires knowledge of professional LPN practice care techniques and medical procedures, medical supplies, sanitation and personal hygiene; and of nutrition.
    • Skill in planning, organizing, and applying nursing care techniques working with clients with mental illness and co-occurring disorders.
    • Able to follow oral and written instructions.
    • Skill in exercising judgment, initiative self-control, patience in following detailed and or written instructions; and in maintaining a sympathetic and harmonious relationship with consumers with mental illness and co-occurring disorders.

    CCBHC Model of Care

    Family & Children’s Services works to heal hurting and abused children, strengthen families, and provide hope and a path to recovery for those battling mental illness and addiction. You will be working in an innovative and dynamic environment using a new transformative model of care Certified Community Behavioral Health Clinic (CCBHC).  This model is characterized by innovative, team-based approach and whole person care for adults and children with a range of complex mental health and substance use challenges.

    Utilizing multi-disciplinary teams, staff work collaboratively to provide a coordinated effort to enhance client recovery. Robust CCBHC services often result in better client outcomes and quality of care due to:     

    1. Increased access to care and crisis services
    2. Expanded traditional community mental health and substance use services.
    3. Added Care Coordination and physical health screening for mental health clients
    4. Greater access to Social Services for clients’ economic and social needs
    5. Increased specialized services for veterans, those most in need, and those impacted by the opioid crisis.
