Job description

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.
Position: PRN PACE Clinic Nurse
Schedule: PRN. 8 hour shifts are during business hours of Monday-Friday from 8am-5pm
Location: Lafayette, CO in a clinic environment
Pay: $42.34 to $50.81 per hour
SUMMARY
The PACE Clinic RN provides skilled nursing care to PACE program participants. They work with other clinic staff in the provision of medical and nursing care, ensuring the highest quality of care in accordance with PACE policy and applicable regulations. While case management and care coordination are a big part of the role, you will also be hands-on with patients regularly.
EDUCATION AND QUALIFICATIONS

  • Must possess a current RN license in the state of Colorado
  • 2+ years working as an RN, highly preferred
  • Experience working with the frail or elderly population, preferred
  • Must successfully perform position-specific competencies prior to assuming participant care as an RN
  • Must successfully pass agency required background checks and drug screening
