Job Summary:

The Staff RN-Ambulatory Clinics works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care. The Staff RN-Ambulatory Clinics systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The Staff RN-Ambulatory Clinics utilizes evidence to drive clinical decisions and practice related to individual and population health. The Staff RN-Ambulatory Clinics establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The Staff RN-Ambulatory Clinics considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The Staff RN-Ambulatory Clinics delegates and supervises care in accordance with the WI Nurse Practice. The Staff RN-Ambulatory Clinics participates in continuous quality improvement initiatives and assumes responsibility for their professional development. Major accountabilities of this role include patient care coordination, direct patient care, patient education, telephone triage/eVisits, and performing/assisting with in-clinic minor procedures. May float to other areas on as needed for operations need. Other duties as assigned.

EXPERIENCE DESCRIPTION:

Clinical experience and/or interest in the care and management of patient populations specific to the area of employment. At least 1 year of relevant nursing experience is preferred.

EDUCATION DESCRIPTION:

Nursing licensure required. Bachelor's Degree in Nursing, preferred.

LICENSURE DESCRIPTION:

Requires current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days of hire.
