Position Description:

Title: Case Manager Reports To: Lead Clinician

Hours: Full-Time (30 hour work week)

FLSA Status: Non-Exempt (Hourly)

Description:

Person in this position will be responsible for linkages to community resources including housing support services to meet the goals and needs of clients in the program. The case manager will assess the strengths and challenges of clients using person-center approach, facilitating a plan of care for each individual. Essential Job Functions include those listed below.

Essential Job Functions:

Provide intensive case management and supportive services to individuals experiencing homelessness with mental health and/or co-occurring substance use disorders.

Conduct client assessments to identify behavioral health, housing, medical, financial, vocational, and social service needs.

Develop individualized care plans in collaboration with clients and other service providers when appropriate that identifying unmet needs, SMART goals and objectives, outlining time frames, and designating responsible parties for service coordination and goal completion

Coordinate and facilitate access to housing resources, emergency shelter, permanent supportive housing, and community-based support services.

Assist clients in obtaining identification documents, benefits enrollment, medical insurance coverage, employment resources, and income support services.

Coordinate care with behavioral health providers, medical providers, probation/parole officers, shelters, social service agencies, and other multidisciplinary partners.

Provide client advocacy to reduce barriers to treatment, housing stability, recovery, and overall wellness.

Conduct outreach, engagement, crisis intervention, and follow-up services to support ongoing client participation and continuity of care.

Monitor client progress toward treatment and service goals and revise care plans as clinically appropriate.

Maintain accurate, timely, and compliant documentation of client contacts, referrals, coordination of care, and service activities in accordance with program standards, HIPAA, and 42 CFR Part 2 requirements.

Participate in multidisciplinary case staffing meetings, supervision, and community collaboration efforts to support integrated service delivery.

Promote trauma-informed, culturally responsive, and recovery-oriented approaches in all client interactions and service coordination activities.

Attend required monthly Guam Homeless Coalition Continuum of Care meetings and/or trainings;

Provide all services in linguistically and culturally appropriate manner, including use of interpreters for those with limited English proficiency;

Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and

Perform all other duties as assigned.

Education:

Graduation from an accredited 4 year college with a degree in Psychology, Social Work,

Community, and Human Services, Rehabilitation Counseling, or a related field.

Experience & Competencies

Most have a valid Guam Drivers License
