About the Role
CareOne Senior Care is looking for a Client Success & Intake Manager to own the full client journey - from that very first phone call through months and years of ongoing care. If you love building relationships, solving problems on the fly, and being the steady hand a family can count on during a hard transition, this role was built for you.
This isn't a task-completion job. It's a relationship-and-outcomes role: your work directly shows up in how many families we help, how long they stay with us, and how supported our caregivers feel every day.
What You'll Do
  • Be the first friendly voice families hear when they reach out for help
  • Conduct in-home assessments and build individualized care plans
  • Serve as the ongoing point of contact for clients and families
  • Conduct in-home Quality Assurance visits to check on service quality and satisfaction
  • Partner closely with the scheduling team on staffing and caregiver matching
  • Support caregiver hiring, onboarding, and ongoing coaching
  • Represent CareOne in the community and with referral partners
  • Help keep the office running smoothly, stepping up to lead day-to-day operations when the Agency Administrator is out
  • Occasionally step in to cover a client care shift directly in the field, such as during a last-minute caregiver call-off

A Note on Availability
This role participates in the after-hours on-call rotation and responds to urgent client and staffing needs during on-call periods. Reliable availability during assigned rotations is a core expectation of the role.
