Job description

We are looking for a Skilled RN to join our clinical team and care for patients in the CSRA/Greater Augusta Metro Area.
Overseeing patient care will include supervisory visits and recertification assessments for our patients who receive care for a chronic, degenerative, medical conditions. Visits will typically last 1 hour - 1.5 hours. Every visit will be paid out as $125 and anything additional (client/care coordination) will be paid at $40 per hour, whichever is more. Every client must have at least one visit per month. Visits for each client can be completed at any time during that 30 day period. Additionally, this position will include patient intake assessments, coordination with physicians, training of field staff, and other similar patient care oversights.
As a Clinical Supervisor, your expertise will be utilized in many ways, such as:
  • Clinical oversight from admissions through discharge of homecare clients and patients
  • Supervision of care management
  • Orientation and ongoing evaluation of homecare caregivers
  • Assurance of clinical skills and competency of caregivers
  • Caregiver development through education and training

Minimum Requirements:
  • Active / Unencumbered Georgia Nursing License (RN) required
  • Minimum experience requirement
    • Three (3) years nursing experience as RN
    • At least one (1) year of home care experience, direct patient care or supervisory
  • Must meet all federal, state, local contract and program requirements, in addition to internal certifications and training, as required
  • Excellent written and verbal communication skills preferred
  • Demonstrated critical thinking skills preferred
  • Supervisory experience required
  • Knowledge of Georgia Healthcare System, including Medicaid waiver programs
  • Pediatric experience preferred
  • Knowledge of Microsoft Office and computer literacy is required
  • Proficiency in the English language is required

Position Requirements:
  • Ability to perform patient visits as scheduled or assigned required
  • Training/ education of field staff required
  • Collaborate with Director of Clinical Services on development of QIs / SOPs, etc
  • Partner with Recruiter(s) collaborating on patient service delivery
  • Assess, evaluate, and develop patient plans of care
  • Coordinate with community partners
