 We are seeking an Outpatient Care Coordinator to support our psychiatric outpatient team, with a primary focus on medication coordination. This role is responsible for managing refill requests, resolving prescription issues, and ensuring timely follow-up with providers and pharmacies. This position works in coordination with front office staff to support scheduling, patient communication, and overall clinic flow, while ensuring documentation is accurate and completed on time.

As a Outpatient Care Coordinator, your responsibilities include: 

  • Welcome and assist clients in a professional and friendly manner
  • When appropriately credentialed, may perform vital signs as directed
  • Assist clients with telehealth appointments
  •  Ensures all medication-related calls are returned within one business day 
  •  Ensures clients receive medications by managing refill requests, resolving prescription issues, and coordinating with providers and pharmacies, without making clinical decisions
  • Coordinates client services in collaboration with outpatient psychiatric providers and staff
  • Communicates with clients regarding appointments, medications, and follow-up needs
  • Works with scheduling staff to arrange appointments
  • Enter and maintain accurate information in the EHR
  • Complete required forms and documentation
  • Uses appropriate forms, both paper and electronic
  • Complete FASAMS admissions and quarterly updates
  • Ensures documentation is accurate, complete, and timely
  •  Perform other duties as assigned



Requirements

PREFERRED:

  •  Experience with scheduling, referrals, insurance authorizations, or care coordination preferred 
  •  Bilingual skills are a plus 
  • Associate's or Bachelor's degree preferred

REQUIREMENTS:

  • High school diploma or equivalent required
  • 1-3 years of experience in a healthcare, behavioral health, or outpatient setting
  • Proficiency in Microsoft Office (Outlook, Word, Excel)
