Job description

Upper Valley Services (UVS) is one of Vermont's designated non-profit agencies serving people with developmental disabilities and adults who are eligible for Adult Family Care.
UVS is seeking a Service Coordinator candidate who is:
  • Detail orientated
  • Familiar with documentation
  • Values teamwork but works well independently too
  • Experience with people with developmental disabilities
  • Works well with deadlines
  • Confident

  • Upper Valley Services is a person-centered program.
  • Service Coordinators meet at least monthly with the shared living provider and participant in their home to support and manage various aspects of the state-required expectations to ensure a healthy environment that meets the participant's goals and needs.
  • Mix of field work and main office in Moretown.
  • Background checks are required
  • Must have valid driver's license and insured vehicle

  • M-F 8am-4pm and occasional evening or weekend availability
  • College degree in Human Services or related field preferred.
