Primary Care Nurse Practitioner
Location: Jersey City, New Jersey
This hospital-based position focuses on caring for medically complex adult patients across the Emergency Department, Intensive Care Unit (ICU), and medical/surgical units.

Responsibilities
  • Perform comprehensive patient evaluations and follow-up visits
  • Manage medically complex adult patients with multiple chronic conditions
  • Develop and implement individualized treatment plans
  • Coordinate care with physicians, specialists, nursing staff, and hospital teams
  • Educate patients and families regarding diagnosis, treatment, and ongoing care
  • Document patient encounters accurately within the electronic medical record
  • Participate in interdisciplinary rounds and quality improvement initiatives
  • Collaborate with care teams to ensure safe discharge planning and continuity of care
Qualifications
  • Active New Jersey Nurse Practitioner license
  • National board certification
  • DEA registration or eligibility
  • Minimum of two years of Nurse Practitioner experience preferred
  • Strong clinical assessment and decision-making skills
  • Excellent communication and interpersonal skills
  • Ability to work independently while collaborating within a multidisciplinary team
Preferred Qualifications
  • Experience in hospital medicine, internal medicine, family medicine, geriatrics, critical care, or management of medically complex patients
  • Experience caring for hospitalized adult patients
  • Bilingual skills are a plus

  • Competitive base salary: $130,000-$160,000 annually (open to discussion based on experience)
  • Annual performance bonus of up to 12.5% of base salary
