Job description

Job Description

We are seeking an RN for Assisted Living and Home Care at our Como Park campus.  This position reports to the AL Clinical Director.  This non-exempt, full-time position will be M-F from 8:30am to 5:00pm. We offer exceptional benefits and great work-life balance.
The posted pay range of this hourly position is $33.65 to $40.48. The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Hiring at the maximum of the range is not typical.
Duties

  • Conducting resident assessments per policy and regulations, including pre-admission, admission, 14-day, quarterly, and change in condition assessments
  • Supervision of unlicensed staff
  • Care plan development and implementation of services to meet resident needs
  • Collaboration with physicians, lab, hospice, and other outside provers
  • Staff education and training
  • Working closely with residents and families to coordinate care
  • Assisting with care conferences when needed
  • Providing back-up to our after-hours triage service

Qualifications

  • The successful candidate must possess a current, unencumbered MN RN license
  • A minimum of two years of direct nursing experience in a long-term care, Home Health, or Assisted Living setting preferred.
  • Excellent communication skills
  • Eldermark experience a plus
