As a Physical Therapist at Heal & Revive Chiro Clinic, you will:

• Develop and implement individualized physical therapy treatment plans aimed at preventing disabilities, alleviating pain, and enhancing patients' functional abilities.
• Evaluate patients to assess their physical capabilities through comprehensive diagnostic assessments, identifying areas for improvement and setting realistic rehabilitation goals.
• Provide hands-on therapeutic exercises, both manual and non-manual, tailored to clients’ specific needs while ensuring their safety and promoting their independence.
• Monitor the progress of patients, adjusting treatment plans as necessary based on their response to therapy.
• Collaborate with the interdisciplinary team to contribute to comprehensive care planning during case conferences, fostering an integrated approach to patient management.
• Document patient care accurately and timely, including initial evaluations, ongoing progress, and discharge summaries, ensuring compliance with regulatory standards.
• Educate patients and their families about physical therapy techniques and procedures, empowering them to participate actively in their recovery process.
• Supervise and mentor Physical Therapy Assistants, ensuring they adhere to prescribed care plans and maintain high standards of patient care.
• Participate in ongoing professional development activities, staying current with the latest evidence-based physical therapy practices.

The primary client population at Heal & Revive Chiro Clinic includes individuals recovering from sports injuries, post-surgical rehabilitation, chronic pain conditions, and neuro-musculoskeletal disorders. The ideal candidate will possess:

• A Doctorate in Physical Therapy from an accredited program.
• Current licensure as a Physical Therapist in good standing in the state of California.
• Current CPR certification.
• A valid driver’s license and automobile insurance, as some home care may be involved.

While not mandatory, the following experiences will enhance your application:

• At least one year of experience in an acute care or home care setting.
• Demonstrated ability to lead and innovate in therapeutic modalities and patient care.
• Familiarity with OASIS documentation and other regulatory requirements specific to rehabilitation facilities.
• Working knowledge of physical therapy methodologies and engagement strategies.
