• Conduct thorough psychosocial assessments of patients, identifying emotional, social, and financial needs to tailor effective care plans.
• Provide individual and group counseling focused on adjustment, grief, and coping mechanisms specific to the hospice context.
• Collaborate closely with interdisciplinary teams, including medical professionals and nursing staff, to integrate holistic care plans that address both medical and psychosocial aspects.
• Facilitate the coordination of post-discharge services through connections with community organizations, ensuring a seamless transition and continuity of care.
• Offer educational resources and emotional support to patients and their families about treatment options and available community resources.

This position primarily focuses on patients receiving hospice care, a sensitive area requiring profound empathy and understanding of end-of-life issues. • A Master's Degree in Social Work from an accredited institution is required.
• Must possess an active state license as a Licensed Independent Clinical Social Worker (LICSW).

• Demonstrated ability in communication and interpersonal skills is crucial for collaborating with diverse teams and supporting patients and families effectively.

While two years of experience in a healthcare or hospital environment is preferred, candidates with relevant internships or training placements in these settings will also be considered. Familiarity with hospice care principles and practices will be especially advantageous.
