Nurse Practitioner/Physician Assistant - Congestive Heart Failure Specialist- (Atlanta)

Northwest Atlanta

Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, Texas, Florida, and Georgia. Ennoble Care's clinicians go to the home of the patient, providing continuum of care for those with chronic conditions and limited mobility. Ennoble Care offers a variety of programs including, remote patient monitoring, behavioral health management, and chronic care management, to ensure that our patients receive the highest quality of care by a team they know and trust. Job Responsibilities
  • Patient Assessment: Conduct comprehensive assessments to identify early signs of CHF exacerbation, enabling timely interventions to prevent hospitalizations.
  • Treatment Planning: Develop and implement personalized treatment plans aimed at achieving maximal doses of GDMT, optimizing heart function, and reducing symptoms.
  • Medication Management: Carefully titrate medications to reach target doses of GDMT, monitor for side effects, and ensure patient adherence to therapy.
  • Patient Education: Educate patients and their families on CHF management, emphasizing the importance of medication adherence, lifestyle changes, and recognizing early symptoms of worsening heart failure.
  • Symptom Management: Proactively manage symptoms through home-based interventions, such as diuretics adjustments and dietary modifications, to prevent acute exacerbations.
  • Coordination of Care: Collaborate with a multidisciplinary team, including cardiologists, home health nurses, and social workers, to provide comprehensive, coordinated care in the home setting.
  • Follow-Up Care: Conduct regular home visits or telehealth consultations to monitor patient progress, adjust treatment plans, and provide ongoing support.
  • Emergency Care: Provide immediate care for acute CHF symptoms at home, reducing the need for emergency room visits and hospital admissions.
  • Patient Advocacy: Ensure patients have access to necessary resources, such as home health services and support groups, to enhance their quality of life.
  • Research and Quality Improvement: Participate in research and quality improvement projects focused on home-based CHF management and the effectiveness of GDMT in reducing hospitalizations and improving patient outcomes.
Qualifications
  • Current NP or PA license in [Your State]
