Job Summary
The Congestive Heart Failure Nurse Coordinator specializes in managing heart failure for patients in and out of the hospital. They create and implement health strategies to promote wellness in congestive heart failure patients. They work closely with healthcare providers to assess and address the self-care and education needs of patients and families. Additionally, they provide different resources for learning.
Essential Functions of the Role
  • You will oversee care for patients with various needs, including mental, physical, and social areas. Your job involves coordinating care plans after discharging patients, working closely with referring doctors on medications and risks.
  • Reviewing financial and outcome data for heart failure patients to improve healthcare delivery.
  • Managing program policies, protocols, and data. Helping with selections on CHF nursing role scope.
  • Leading projects, giving input on clinical outcomes and financial indicators, and analyzing the effects of practices and environment.
  • Partnering with management to address any clinical or professional issues.
  • Partnering with educators and managers to create a supportive learning environment.
Key Success Factors
  • Familiarity with patient care coordination, resource allocation, and usage management is beneficial.
  • Skills in patient care coordination and assessment are necessary.
  • Exceptional verbal and written communication abilities are imperative.
  • Capable of closely monitoring, evaluating, and documenting patient progress in conjunction with a care plan.
  • Efficient in facilitating patient access to community resources.
  • Capable of fostering collective relationships with both internal medical staff, external medical professionals, partners and organizations.
