Their goal is to improve patient outcomes by providing guidance, advocacy, and education tailored to the unique needs of those with CHD.

What are some common challenges faced by CHD case managers when coordinating care for clients with complex needs?

CHD Case Managers often encounter challenges such as navigating fragmented healthcare systems, managing communication among multiple providers, and addressing social determinants of health that may impact clients' access to care.
