Essential Functions

  • Pull up prior medical record charts, route according to surgery schedule procedure or medical record requests and refile as necessary.
  • Prepare chart packs, print preference cards and keep chart assembly inventory stocked. H&P's are placed in patient care binders prior to surgery.
  • Monitors dictation systems to ensure each patient has a dictated operative report and notifies physician and management of pending reports. Performs QA chart analysis to ensure diagnostic reports and other medical record deficiencies are routed to the appropriate physician for completion. Keeps management aware of physician issues and deficiencies.
  • Coordinates receipt of dictation between transcription company and facility through mytranscribe Intrascript printing application; sends out carbon copy reports. Handles any issues between the entities that may arise and informs management of any issues.
  • Ensures incoming reports, charts and other loose paperwork are filed on a daily basis once finalized.
  • Understands and is able to process release of information requests i.e. subpoenas according to departmental procedures and when required makes court appearances as the Medical Records Custodian.
  • Prepares medical records to be scanned into Laserfiche while making sure that the chart is completely scanned, images are present and visible prior to destruction of the record.
  • Utilize and understands applications i.e. Paragon, AdvantX, DVI, Laserfiche, Mytranscribe in order to log and track medical record deficiencies and complete daily tasks.
  • Demonstrate ability to troubleshoot minor PC, printer, and fax problems and/or reports to appropriate personnel.
  • Demonstrate ability to assist physicians with record completion policies, dictation system and/or other H.I.S. applications without assistance.

Required Education and Experience: High School graduate or GED. One or two years related experience
