Job description


Matrix Home Care is currently seeking compassionate, experienced, and motivated Registered Nurses (RNs) to join our growing Home Health team in Bradenton, Florida. This position focuses heavily on Start of Care (SOC) admissions and skilled nursing visits for patients receiving care in the home setting.
We are seeking clinicians who are flexible, dependable, patient-focused, and comfortable working independently in the field.
Responsibilities
  • Perform Start of Care (SOC) admissions and comprehensive patient assessments
  • Complete OASIS documentation accurately and timely
  • Conduct recertification, resumption of care, and routine skilled nursing visits
  • Develop and update individualized care plans
  • Provide patient and caregiver education
  • Coordinate care with physicians, therapists, case managers, and interdisciplinary teams
  • Maintain compliance with Medicare, state, and company guidelines
  • Participate in on-call rotation as needed

Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.

Requirements
Qualifications
  • Active Florida RN license required
  • Minimum 1 year of RN experience required
  • Home Health and SOC experience strongly preferred
  • OASIS documentation experience required
  • Medicare-certified home health experience preferred
  • EMR experience required; Kantime experience preferred
  • Strong clinical assessment and time management skills
  • Reliable transportation, valid driver's license, and auto insurance required
