Provide Comfort, Support, and Meaning When It Matters Most
Dependable Hospice is seeking a compassionate and dependable PRN Hospice Chaplain to join our growing Phoenix hospice team. This position provides spiritual and emotional support to hospice patients and their families while honoring each individual's personal beliefs, values, and traditions.
What You'll Do
  • Provide spiritual and emotional support to hospice patients and families
  • Complete spiritual assessments and develop individualized plans of care
  • Offer prayer, counseling, presence, and other spiritual support based on patient and family preferences
  • Provide support during end-of-life transitions, grief, and bereavement
  • Collaborate with nurses, social workers, physicians, CNAs, and other members of the hospice interdisciplinary team
  • Participate in interdisciplinary group (IDG) meetings as needed
  • Maintain timely and accurate documentation of visits and services
  • Coordinate with community clergy or spiritual resources when appropriate

  • Previous hospice, healthcare, or end-of-life spiritual care experience preferred
  • Education or training in theology, ministry, pastoral care, or a related field
  • Strong communication and interpersonal skills
  • Ability to provide compassionate, nonjudgmental support to individuals of diverse faiths and beliefs
