Experience/Qualifications:
  • Licensed Physical Therapist (Required)
  • Open to PTs of all experience levels
Key Responsibilities:
  • Effectively screen and evaluate patients with cognitive and ADL physical functioning disorders, creating individualized treatment plans that meet regulatory and clinical practice standards.
  • Utilize evidence-based practice and standardized assessments to support clinical interventions.
  • Provide rehabilitative, skilled treatment interventions including ADL retraining, therapeutic activities, and assistive device training.
  • Assess patient progress and adjust treatments as necessary to meet goals.
  • Work closely with families, caregivers, and the healthcare team to ensure comprehensive patient care.
  • Communicate progress effectively with the healthcare team and document outcomes according to regulatory standards.
  • Provide discharge summaries and recommendations for continued care.
