The Change Management Program Manager owns the adoption engine within the Enterprise Transformation Office, using change management technology, analytics, and reinforcement campaigns to drive and sustain adoption at enterprise scale. This role designs personalized change journeys, executes reinforcement and engagement campaigns, and builds the measurement capability that tells the organization whether change is taking hold. Working closely with the Director of Change Management and the Senior Change Management Specialist, the Change Management Program Manager turns change strategy into scalable, data-informed adoption and equips Premise Health with reusable enablement and measurement capability for future transformations.
This is a full time, remote Change Management Program Manager role.
What You'll Do
• Administer and optimize the change management technology platform, including delivery within Microsoft environments, to enable scalable adoption.
• Design and execute personalized change journeys, nudges, and reinforcement campaigns targeted to specific audiences and impacted groups.
• Develop audience segmentation and targeting strategies to ensure the right messages and enablement reach the right people at the right time.
• Build, track, and report adoption and engagement metrics; develop dashboards and surface behavioral insights to leadership and workstream teams.
• Partner with the Senior Change Specialist to align adoption journeys and reinforcement with workstream readiness and change plans.
• Design and curate enablement content, training reinforcement, and continuous learning journeys that build user capability and sustain adoption.
• Establish feedback mechanisms (e.g., pulse surveys) and translate results into recommendations that improve adoption outcomes.
• Support enterprise adoption governance by maintaining measurement frameworks, readiness dashboards, and sustainment reporting.
• Build reusable adoption, enablement, and measurement assets that scale across initiatives and strengthen long-term enterprise change capability.
What You'll Bring
• Bachelor's degree preferred or equivalent work experience.
• Change management, adoption, or related certification preferred (e.g., Prosci); change management technology or analytics credentials a plus.
• 7+ years of experience in change adoption, change enablement, digital adoption, communications analytics, or a related discipline.
• Hands-on experience with change management, digital adoption, or engagement platforms and with building adoption or engagement dashboards.
• Familiarity with change management methodologies (e.g., ADKAR, Prosci, Kotter) and adoption measurement practices.
• Strong analytical and data visualization skills, with the ability to translate adoption and engagement data into clear insights and recommendations.
• Proficiency with change management or digital adoption technology and Microsoft collaboration tools.
• Skilled in campaign design, audience targeting, and reinforcement content development.
• Collaborative, detail-oriented, and able to manage multiple campaigns and priorities in a fast-paced transformation environment.

Premise provides its reasonable and genuinely expected range of compensation for this job of $90,000.00 - $125,000.00 annually. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
For individuals living in Washington or Colorado: Premise offers the following paid time off benefits. Employees will be included in the company's Open Time Off plan and will receive 10 paid holidays or an equivalent bank of hours aligned to schedule throughout the calendar year. Paid sick leave is satisfied by the paid time off accrual, detailed above.
