The Care Team Hospice is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing team!! This is an opportunity to make a meaningful difference by providing hands-on personal care and comfort to hospice patients while supporting families through life's most important moments.

As a Hospice CNA, you'll work alongside an experienced interdisciplinary team to help patients maintain dignity, comfort, and quality of life in their homes and care facilities. Key Responsibilities

Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.

Observe, document, and promptly report changes in patient condition to the supervising nurse.

Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.

Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.

Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

Qualifications

Active Certified Nursing Assistant (CNA) certification

Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Field Role Requirements

Valid driver's license, reliable transportation, and current automobile insurance.

Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
