Position Overview We're seeking an experienced Home Care Supervisor to lead our new CCP vertical while supporting existing operations. This role offers significant growth opportunities.


Key Responsibilities:

Administrative Duties:

· Answer and transfer phone calls

· Provide quality customer service

· Maintain filing systems

· Lead the establishment of CCP vertical

· Build relationships with CCUs and community partners

· Manage incoming client referrals

Staffing Responsibilities:

· Recruit, hire, and terminate employees

· Conduct pre-service and in-service training

· Coordinate client care (caseload of up to 50 clients)

· Communicate with vendors regarding documentation and service deviations

Data Entry & Operations:

· Enter and maintain employee/client data accurately

· Manage client assessments and visit reports

· Ensure compliance with IDoA requirements

· Integrate CCP operations with existing home health services

Supervisory Responsibilities:

· Create/update client and employee folders

· Conduct quarterly evaluations

· Process timesheets and manage payroll reports

· Submit accurate payroll data

· Develop operational procedures for CCP services

Customer Service:

· Refer clients to appropriate programs

· Assist with service applications

· Respond to inquiries within 24 hours

· Resolve client/employee issues efficiently

Required Qualifications:

· CCP supervision experience is a must.

· Strong written/verbal English communication ( Knowledge of Spanish or any other language a plus)

· Exceptional organizational abilities

· Computer proficiency (Microsoft Office)

· Prior office experience

· Must pass Homecare Supervisor Certification (3 months)

Preferred Qualifications:

· Experience with the WellSky Personal Care platform (formerly Clear Care)

· IDOA compliance knowledge

· Home care operations experience

· CCP program experience

· Track record in scaling healthcare operations
