Registered Nurse (RN) Lead for Chronic Care Management

Summit Spine and Joint Centers (SSJC) is a rapidly growing, multi-state spine and joint care organization providing integrated clinical, surgical, and imaging services. With clinic locations across Georgia, North Carolina, South Carolina, and Tennessee, our care teams include IPS in North Carolina and Savannah Pain Management in Georgia, all operating as part of the SSJC organization. We are committed to collaboration, high-quality patient-centered care, and supporting our teams as we continue to expand. We are seeking motivated, qualified professionals to join us in delivering exceptional care across our growing network.

Summit Spine and Joint Centers is seeking an experienced full-time Registered Nurse (RN) to serve as a lead within our Chronic Care Management (CCM) team. This position will support the coordination and ongoing management of care for patients with chronic conditions while providing clinical guidance and leadership to the CCM team.

The ideal candidate is an experienced RN with excellent communication, clinical judgment, and organizational skills. Previous Chronic Care Management, case management, telephone triage, or nursing leadership experience is strongly preferred.

Essential Duties and Responsibilities
  • Provide clinical leadership and support to the Chronic Care Management team.
  • Support the coordination and ongoing management of care for patients enrolled in the CCM program.
  • Communicate with patients regarding their care plans, health concerns, medications, appointments, and other care needs.
  • Review patient information and identify concerns requiring additional clinical intervention or escalation.
  • Provide appropriate nursing guidance within the scope of RN practice.
  • Assist with complex patient situations and clinical questions escalated by members of the CCM team.
  • Coordinate with providers and other members of the healthcare team to support continuity of patient care.
  • Perform patient outreach and follow-up by telephone as needed.
  • Maintain accurate, timely, and thorough documentation within the electronic medical record (EMR).
  • Help promote consistent workflows, documentation standards, and quality patient interactions across the CCM team.
  • Serve as a resource and provide guidance to other members of the CCM team.
  • Maintain patient confidentiality and comply with HIPAA and organizational policies.
  • Perform additional duties as assigned based on departmental and organizational needs.
Minimum Qualifications
  • Current and unrestricted Registered Nurse (RN) license.
  • Minimum of five (5) years of professional nursing experience.
  • Clinical experience in ICU, Med-Surg, Emergency Room (ER), or a comparable acute-care environment.
  • Strong clinical assessment and critical-thinking skills.
  • Excellent verbal and written communication skills.
  • Strong organizational skills and attention to detail.
  • Ability to communicate effectively and professionally with patients, providers, and team members.
  • Comfortable working extensively within an electronic medical record (EMR) system.
Preferred Qualifications
  • Previous Chronic Care Management (CCM) experience.
  • Previous hospital case management or care coordination experience.
  • Previous telephone triage or RN phone-line experience.
  • Previous leadership, team lead, or supervisory experience.
  • Experience managing or coordinating care for patients with chronic or complex medical conditions.
