Job Summary
This position offers a n opportunity to part of a dynamic PHP/IOP program located at the Institute of Living. The case manager will be directly involved with assisting patients locate resources in the community and on campus, apply for additional benefits, access local transportation services to facilitate program adherence and complete paperwork for leave status.
Qualifications:
  • Bachelor Degree in Human Services
  • Ability to obtain Public Service License required
  • Demonstrated competence including knowledge and behaviors to meet age-specific needs of the client population
  • Experience working in related human service setting with similar age group
