The Community Living Case Manager provides community-based case management, service coordination, advocacy, and support to individuals living independently. This role will help individuals achieve personal goals, access services and benefits, strengthen independent living skills, and maintain health, safety, and community connections. The Community Living Case Manager develops and monitors person-centered service plans, coordinates support, advocates for individual needs and rights, and ensures services are delivered in accordance with agency, DDS, and regulatory requirements.
Minimum Qualifications:
  • High School Diploma or General Equivalency Diploma (GED) required
  • Must be at least 18 years of age
  • Associate’s or Bachelor’s Degree in Human Services or related field preferred, or 2-4 years of related experience
  • Experience with case management, service coordination, DDS-funded services, person-centered planning, documentation, advocacy, or community resource navigation strongly preferred.
  • Valid driver’s license, safe driving record, personal vehicle, and current motor vehicle liability insurance required
  • Ability to successfully pass required criminal background checks

Job Responsibilities:
*Responsibilities are based on the needs, goals, and preferences of individuals and families and may vary accordingly*
  • Provide ongoing case management and service coordination for individuals with intellectual and developmental disabilities who live in the community.
  • Develop and maintain positive, respectful, and person-centered relationships with individuals served, families, guardians, DDS staff, providers, and community partners.
  • Assist individuals in identifying personal goals, preferences, strengths, support needs, and desired outcomes related to independent living, community participation, health, relationships, employment, education, and daily life.
  • Participate in the development, implementation, monitoring, and review of Individual Service Plans, Plans of Care, assessments, and related service documentation as required.
  • Coordinate referrals and link individuals to appropriate community resources, public benefits, housing supports, transportation, health care, behavioral health services, vocational supports, social opportunities, and other needed services.
  • Monitor service delivery through regular contact with individuals served, home and community visits, provider communication, team meetings, and review of progress toward goals.
  • Advocate with and on behalf of individuals to ensure access to services, protection of rights, informed choice, dignity, inclusion, and self-determination.
  • Assist individuals with problem solving, skill building, appointment coordination, benefit-related follow up, communication with providers, and navigation of community systems.
  • Identify changes in needs, risks, service gaps, or health and safety concerns and communicate concerns promptly to the supervisor and appropriate team members.
  • Support crisis planning, transition planning, incident follow-up, and coordination of additional support when needed.
  • Maintain timely, accurate, and complete documentation, case notes, service records, reports, and required forms in accordance with agency, DDS, funding, and regulatory requirements.
  • The Case Manager may be required to transport individuals using their personal vehicle.
