Job description

Job Summary
The Therapeutic Case Manager provides case management, psychoeducation, and clinical recommendations to a caseload. The Therapeutic Case Manager works with clients to develop treatment plans and implement therapeutic interventions to support clients in reaching established goals across settings, including in homes, community, and the agency. The Therapeutic Case Manager coordinates services, including making referrals to additional services when needed (including in mental health crises), collaborates with other service providers, maintains appropriate documentation of services provided, and may provide transportation for clients. The Therapeutic Case Manager follows established guidelines of ethical practice in accordance with Vermont state law.
The case manager working in the Children, Youth, and Family Services Program will support clients aged 0-22 and their families. The case manager provides services across environments as needed to reach therapeutic goals including in client's homes, at the agency, and in the community. They will support families in coordinating services, building resources, and reaching treatment goals in partnership with community partners including academic environments, the Department of Children and Families, and other agencies as needed.
