About Us

Springbrook Psychiatric Hospital is a 66-bed inpatient mental health facility located in Hernando County, Florida. We offer 24-hour psychiatric services provided by licensed professionals in various disciplines. Our hospital offers a variety of programs that can be tailored to patient needs, including an older adult program, dual diagnosis treatment, and general psychiatric care.

Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our interdisciplinary treatment team. The Case Manager provides case management services to patients during their hospitalization, including evaluations, assessments, consultations, and discharge planning. As a Case Manager, you will work closely with patients, families, and the treatment team to develop and implement comprehensive discharge plans that meet each patient's unique needs.

Responsibilities

  • Interview patients, families, and significant others to assess admission factors, emotional and psychological problems, and available support systems

  • Formulate and implement a clinically sound plan of care for patient discharge

  • Assess and follow up on issues concerning patient rights, including the Baker Act and emergency admissions procedures

  • Educate patients and families about legal rights and provide education on discharge planning and aftercare

  • Coordinate discharge planning with families, the interdisciplinary treatment team, and community resources

  • Participate in the development of Case Manager's role within the department and hospital

  • Complete and utilize all relevant documentation

  • Actively contribute to a positive and collaborative work environment

Requirements

  • Associate's Degree in behavioral science

  • Six months of clinical experience in a psychiatric setting, or two years of combined post-high school education, training, and experience

  • Current CPR and First Aid Certification (if client contact is part of the job)

  • Case Management Certification preferred

  • HIV/AIDS training required every two years

  • Knowledge of clinical theory and specific techniques of intervention, federal, state, and local laws related to patient rights, and community and discharge resources

  • Skilled in utilizing referral resources and displaying sound judgment and reasoning

  • Ability to work with a multi-disciplinary team approach and respond appropriately to emotionally stressful situations

  • Knowledge of safety standards and emergency care procedures
