Description

American Traveler is hiring an experienced RN Case Manager for a hospital-based case management position requiring a GA or compact RN license and a minimum of 3 years of experience.

Details
  • Hospital-based case management unit
  • Day shift schedule, Monday through Friday, 8:00 AM – 4:30 PM (5x8-hour shifts)
  • No weekend or on-call requirements
  • Floating within the facility and scope of practice as needed
  • 12-week contract
Requirements
  • Active GA or compact RN license required
  • Minimum 3 years of RN case management experience required
  • Two supervisory references (manager level or above) highly preferred; charge nurse references are not accepted
  • Copies of all relevant certifications required for consideration
Additional Information
  • Responsibilities include assessing and driving discharge planning, coordinating appropriate level of care, monitoring length of stay, identifying barriers to discharge, and adhering to Medicare regulatory guidelines (patient choice, IMM notices, and MOONS)
  • Treatment plans must be initiated, evaluated, and re-evaluated daily
  • No special scheduling requests, alternate shifts, or schedule modifications are permitted
  • RTO maximum is 7 days; RTO cannot be requested during the first week of the assignment
  • Holiday coverage is required: if one or two major holidays fall within the assignment period, the traveler is expected to work all of them; if three major holidays fall within the assignment period, the traveler is expected to work two — major holidays include Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, and New Year's Eve/New Year's Day
  • Travelers who have previously worked at any facility in this health system must have been away for a minimum of one full year before returning as a traveler
  • Local candidates are accepted
  • First-time travelers are not accepted for this position
