RN (Registered Nurse), Case Manager

City/State: Jackson, Mississippi

Shift: 8H Days 8 hr days, 08:00:00-16:00:00

Contract Length: 13 weeks

Job Description:

The Case Manager is responsible for coordinating patient care and discharge planning for a variety of patients within a Level 1 trauma hospital. The role requires experienced case management with a focus on acute inpatient care and minimal training provided on the unit.

Responsibilities:

  • Coordinate care and discharge planning for 15-20 patients per day across all inpatient units.
  • Work collaboratively with a team of 15 full-time and 17 part-time social workers.
  • Consult with Respiratory Therapy and Pharmacy as needed for discharge planning.
  • Utilize EPIC for charting and manage patient documentation efficiently.

Requirements:

  • Minimum 2 years of Acute Inpatient Medical Case Management experience, including 1 year in a large busy hospital ED (500+ beds, LEVEL 1/ACADEMIC Medical Center).
  • Recent acute care discharge planning experience for patients with diverse medical diagnoses is required.
  • Proficient in EPIC charting and possess strong computer skills.
  • BLS Certification required.
  • Previous travel experience in a Level 1 facility within the last 3 years.
