Job Title: RN Field Case Manager

Are you an experienced Registered Nurse with Case Management/Pediatrics experience looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered "yes" to any of these questions - this is the position for you!


Position Summary:

As the RN Field Case Manager, you would be responsible for providing care coordination, medical/disease education and outreach services to participants. 


Hours for this Position: Monday-Friday 8:00am-5:00pm

More Insight of Daily Responsibilities: 

  • Identify care needs and goals based upon assessment findings, analysis of claims data, provider input, participant goals, etc.
  • Establish care plan in conjunction with participant, caregiver/parents, and providers
  • Convene/facilitate Interdisciplinary Care Team meetings
Qualifications

What We Look For:

  • Clear and active RN license in the state of IL
  • BSN required
  • 3+ years of Case/Disease Management experience
  • 3+ years of clinical pediatric experience
