These individuals often have backgrounds in social work, counseling, or other healthcare-related fields. They work with patients, families, and healthcare providers to develop care plans, facilitate access to services, and help ensure the best possible outcomes. Their responsibilities may include assessing patient needs, advocating for resources, and monitoring progress throughout the care process.

How does a case manager non RN collaborate with other healthcare professionals to ensure effective patient care coordination?
