Job Title: Adult Nurse Case Manager

Profession: RN Specialty: Case Management

Duration: 13 weeks Shift: Day Hours per Shift: 8:30 AM - 5:00 PM

Experience: Minimum 2 years RN experience License: Active RN License Certifications: BLS

Must-Have: Current in-patient Case Management experience; familiarity with EPIC and post-acute facility referral platforms

Description: The Adult Nurse Case Manager will coordinate care for a select caseload of patients from triage through discharge. The role involves care coordination during hospital management and ensuring safe transfer to home. The Case Manager proactively communicates with payors to ensure approval of treatment plans and required resources. This position requires maintaining knowledge of clinical systems and fiscal data related to specific patient populations. The Case Manager will collaborate with a multi-disciplinary team and patients or their families to ensure effective transition planning. This includes coordinating transitions to post-acute settings, such as nursing facilities and rehabilitation centers. The role requires managing patients based on departmental procedures while maintaining clinical safety and quality. Additionally, the Case Manager will enter and maintain patient information in departmental systems, adhering to operational guidelines. Please note that active CPR certification is required for this position; a Med Test is not necessary.
