Location: Mission Viejo, California (Onsite)

• Full-Time (Standard Hospital Schedule)
• Weekend Rotation Required
• Floating: Mission Viejo & Laguna Facility
The Registered Nurse (RN) - Case Manager is responsible for coordinating patient care, discharge planning, and utilization management in an acute care setting.
This role requires strong care coordination skills and experience managing complex patient cases.
Must-Have Qualifications
• Minimum 2+ years of acute care Case Management experience
• Experience in Care Coordination (Required)
Active California RN License (Required by start date)
Travel experience required (No first-time travelers - HARD STOP)
• Strong knowledge of case management workflows and discharge planning
Preferred Qualifications
• Experience in acute care hospital settings
• Strong communication and interdisciplinary coordination skills
• Familiarity with hospital case management systems
