Location: Monroe, NJ 08831

About the Position
We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to provide exceptional in-home clinical oversight. This field-based role is ideal for an organized, independent RN who enjoys building meaningful relationships with clients while delivering personalized, high-quality care in the comfort of their homes.
Responsibilities
  • Conduct comprehensive in-home nursing assessments.
  • Develop, implement, and oversee individualized plans of care.
  • Obtain and maintain physician orders in compliance with New Jersey regulations.
  • Monitor client conditions and promptly communicate significant changes.
  • Complete accurate and timely clinical documentation.
  • Perform Home Health Aide supervisory visits and ensure quality of care.
  • Provide clinical guidance and support to Home Health Aides and Nurses.
  • Promote client safety, independence, comfort, and overall well-being.
Qualifications
  • Active New Jersey Registered Nurse (RN) license.
  • Minimum of 2 years of recent RN experience in acute care, home care, or case management.
  • Recent experience with:
    • Ventilator and tracheostomy care
    • G-tube/PEG tube management
    • IV infusions
    • Wound care
  • Current BLS Certification.
  • Current physical examination (within the past year).
  • Current PPD or QuantiFERON test (within the past year), or chest X-ray completed within the past five years, if applicable.
  • MMR immunization documentation.
  • Valid driver's license and reliable transportation.
  • Authorized to work in the United States.
Compensation
  • RN Assessment: $150 per visit
  • RN Aide Supervision: $125 per visit
  • Hourly Clinical Visits: $55 per hour
  • Weekday On-Call: $50 per day (Monday-Friday, 5:00 PM-9:00 AM)
  • Weekend On-Call: $110 per day (Saturday 8:00 AM-Monday 9:00 AM)
