JOB SUMMARY AND PURPOSE

The MSW Lead provides clients with the highest levels of acuity and co-morbidities individualized care through access to service linkages, counseling, treatment planning, and various crisis intervention methods in order to prepare individuals for further success. The MSW Lead supervises the services provided to clients who have several psychological co-morbidities that must be managed appropriately for a successful re-entry into society. As the Lead Case Manager, this position is responsible for managing most complex client cases. Additionally, coaches and mentors junior staff. The position requires a combination of administrative/office duties and field/outreach work.

DUTIES AND RESPONSIBILITIES

  • Assigns and monitors case load of team:
    • Review daily referrals and screenings of potential clients.
    • Designates clients to case manager for intake and ensures all supporting documents are verified
  • Conducts comprehensive screenings and assessments with clients to collect functional, environmental, financial, employment, housing, educational, and health information, as appropriate, to develop a case plan
    • Develops support systems to meet client needs, coordinates and facilitates move-in activities, rental negotiations, and service referrals as required
    • Makes home and field visits to assigned clients weekly
  • Monitors case managers on a regular basis to determine quality and effectiveness of services provided
    • Review cases with Program Supervisor/Manager and Case Management Team
    • Audit case file on a regular basis for completeness and accuracy, and compliance with regulatory requirements (HIPAA, local law enforcement requirements)
    • Conducts crisis intervention as necessary
  • Assists case managers by identifying and coordinating a variety of available services necessary for clients and families to maintain/obtain self-sufficiency and family stabilization
    • Makes referrals to available community providers for therapeutic, social, educational, and medical resources, as appropriate
    • Provides support to case managers, clients and families through ongoing case conferencing, counseling, and advocacy
  • Provides training, supervision, and problem-solving guidance for Case Management Team in close consultation with Clinicians
  • Develops, performs, and assists with workshops pertaining to program services that meet client needs
  • Attends Court as the client liaison as part of case management.
  • Participates and assists with outreach events
  • Other duties as assigned

ESSENTIAL DUTIES:

  • Services for this position must be provided on-site. As a result, the positions supervised by this job description are on-site, and supervisors / managers / leaders must be on site to provide proper direction and oversight.
  • Assign and Monitor case load of Case Management Team.
  • Conducts comprehensive screenings and assessments to collect information needed to develop complex case plans, develops plans for clients.
  • Monitors the progress of clients of all Case Management Team, conducts intervention as needed.
  • Assists Case Management Team by appropriate referrals, conferences and counselling.
  • Reviews case files for accuracy and regulatory compliance
  • Trains Case Management Team, is a resource for problem solving guidance
  • Plans and participates in workshops and outreach events
  • Attends court with clients to act as client advocate and liaison with regards to case management
  • Collaborates with other service providers to link participants to community resources and supportive services.
  • Coaches and mentors junior staff
  • Other duties as required and may be subject to change

Qualifications

REQUIREMENTS:

  • Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
  • Ability to perform under circumstances of possible emotional stress and conflicts dealing with difficult, uncooperative, and potentially aggressive clients as VOALA serves the neediest of the needy in our programs which may include clients with homelessness, mental-illness, substance abuse, recently released from incarceration and other challenges.

EDUCATION:

  • Master's degree in social work, psychology, or counseling from fully-accredited college or university.

EXPERIENCE:

  • Minimum of 3 years of complex clinical experience in program(s) serving homelessness, addictions, under-served populations or prison re-entry/diversion
  • Proven leadership qualities.

PREFERRED QUALIFICATIONS:

  • Knowledge of/experience with substance abuse services in L.A. County
