Registered Nurse Case Manager – Hospice

Location: Barre, Vermont

Pay Rate: $2,350–$2,560/Week

Contract: 16 Weeks

Schedule: Day Shift | Monday–Friday

Experience: Minimum 1 Year of Hospice/Home Health RN Experience Required

About the Role

We are seeking a compassionate Registered Nurse Case Manager – Hospice for a 16-week contract opportunity in Barre, Vermont. This role is ideal for an experienced hospice and home health nurse dedicated to delivering high-quality, patient-centered end-of-life care. The RN Case Manager will coordinate individualized care plans, manage symptoms, provide family education, and collaborate with the interdisciplinary team to ensure comfort, dignity, and quality of life for patients receiving hospice services in their homes.

Responsibilities
  • Perform comprehensive patient assessments and provide skilled hospice nursing care in the home setting.

  • Develop, implement, and manage individualized hospice care plans while coordinating services as the primary RN Case Manager.

  • Monitor and manage pain, symptoms, medications, and changes in patient condition to promote comfort and quality of life.

  • Educate patients, families, and caregivers on hospice care, disease progression, medications, and end-of-life support.

  • Maintain accurate documentation using Homecare Homebase (HCHB) and complete OASIS documentation in compliance with regulatory standards.

Requirements
  • Active Vermont Registered Nurse (RN) License required.

  • Minimum 1 year of Hospice and Home Health RN experience required.

  • Current BLS/BCLS Certification required.

  • OASIS documentation and Home Health experience required.

  • Experience with Homecare Homebase (HCHB) preferred.
