Are you a compassionate, solutions-oriented professional with aย bachelorโ€™sย degreeย in social work, psychology,ย counselingย or a related field? Do you thrive in a collaborative, fast-paced environment where your clinical insight and case managementย expertiseย can make a meaningful impact?ย 

U.S.ย VETSโ€”the nationโ€™s largest nonprofit dedicated to ending veteran homelessnessโ€”is hiring aย Case Manager IIย to provide comprehensive support to veterans overcoming housing instability, trauma, mental health challenges, and other barriers to long-term stability.ย ย 

Whatย Youโ€™llย Doย 

As aย Case Manager II, you will provide trauma-informed, strengths-based case managementย to veteransย Responsibilities include:ย 

  • Conduct thorough client assessments and develop individualized service plans (IAPs) addressing housing, employment, education, recovery, legal, financial, medical, and mental health needs.ย 

  • Coordinate andย monitorย services with the VA, landlords, employers, treatment providers, and community resources to help clients meet their goals.ย 

  • Provide regular one-on-one meetings with clients and conduct home visits (project-based and scattered-site housing).ย 

  • Conduct home visitsย atย project-based units or scattered sites toย ensure health and welfare of clientsย 

  • Transport clientsย in company vehiclesย toย appointments, interviews, courtย appearancesย and other meetingsย as needed.ย 

  • Facilitate life skills groups and/or therapeutic groups tailored to specialized veteran populations (e.g., female veterans, justice-involved veterans, families with children).ย 

  • Assistย with crisis intervention, urgent client matters, and after-hoursย support as needed.ย 

  • Document all servicesย and critical incident reportsย inย aย timelyย and professional fashion usingย MCR, and otherย electronicย clientย recordย systems.ย 

  • Conductย randomย drug and alcohol screenings and ensure compliance with program rules and service plans.ย 

  • Support reporting, audits, and file reviews in collaboration with the Program Coordinator and Director of Behavioral Health.ย 

Programs You May Supportย 

This role may be assigned to one or more of the following programs:ย 

  • Transitional Housing: Help veterans prepare for and transition into permanent housing.ย 

  • SSVF (Supportive Services for Veteran Families): Deliver rapid rehousing and homelessness prevention services.ย 

  • Permanent Housing (HUD-VASH, etc.): Focus on housing stabilization and long-term support.ย 

  • Workforce Development:ย Assistย clients with employment plans, resumes, and job readiness.ย 

  • Justice-Involved Veterans: Provide pre-release and post-release support forย veteransย exiting incarceration.ย 

  • Aftercare and Counseling Services: Offer ongoing case management andย facilitateย group or individual therapy (if licensed or certified).ย

Required Qualificationsย 

  • Bachelorโ€™s degree in social work,ย psychology,ย counseling, or related fieldย 

  • Atย leastย twoย yearsย ofย workย experience in case management orย other roles in theย social servicesย arenaย ย 

  • Ability to pass background checks if assigned to programs involving incarcerated or recently released clientsย 

  • Strong communication, documentation, and organizational skillsย 

  • Proficiencyย in Microsoft Office and electronic records systemsย 

  • Valid and currentย driverโ€™s licenseย with clean driving recordย toย meet company insurance requirements; ability toย completeย and passย a driver training courseย ย 

  • Not requiredย but a definiteย PLUS!ย 

  • Experience working with homeless,ย veteranย or justice-involved populationsย ย 

  • Certification in Substance Abuse Counseling (e.g., CCAPP, CAADE)ย 

  • Experienceย facilitatingย groups or life skills classesย
