What You’ll Do

As a Home Health RN, you will serve as the clinical leader for your assigned patients, coordinating and delivering skilled nursing care in the home setting.

Key responsibilities include:

* Conducting comprehensive nursing assessments and developing individualized Plans of Care
* Managing a caseload of patients from admission through discharge
* Performing start-of-care visits, routine visits, recertifications, and discharges
* Evaluating physical, psychosocial, environmental, and functional patient needs
* Collaborating with physicians, caregivers, and interdisciplinary team members to ensure continuity of care
* Monitoring patient progress and adjusting care plans as needed
* Educating patients and families on disease management, medications, and self-care strategies
* Identifying early changes in condition and intervening to prevent avoidable hospitalizations
* Documenting all clinical findings, interventions, and outcomes accurately and timely
* Ensuring safe, high-quality, patient-centered care in the home environment

What We’re Looking For

* Current, unrestricted Massachusetts Registered Nurse (RN) license
* Minimum of 1 year of clinical nursing experience
* Home care is strongly preferred
* Valid driver’s license, reliable transportation, and current auto insurance
* Ability and willingness to travel within the Springfield service area (up to 50%)
* Strong communication, assessment, and critical thinking skills
* Physical Requirements - Ability to sit, stand, bend, lift, and move intermittently. Must be able to lift 50–100 lbs.
