POSITION SUMMARY: The Case Manager works in collaboration with other health care professionals to determine and meet the needs of the patients. The Case Manager has the responsibility and accountability for planning continued care and discharge planning through the nursing process. Education: Graduate from an accredited school of nursing. License/Certifications: Registered Nurse licensed to practice in NYS Experience: Previous experience in case management preferred. NYS/PRI/Screen trained. 1-2 years clinical experience preferred. Salary Range: $27.81 - $48.94
