Hospice Registered Nurse (RN) Case Manager – Full-Time | Vernon Hills, IL

At JourneyCare Hospice, we do things differently. We prioritize manageable caseloads, strong interdisciplinary support, and giving our nurses the time they need to truly care for patients and families.

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Vernon Hills area.

Schedule & Territory
  • Territory: Vernon Hills, 60061 and surrounding communities.
  • Monday–Friday, 8:30 AM – 5:00 PM. No On-Call.
  • $80,000 – $97,000 annually (based on experience)
  • 401(k) with company match

Key Responsibilities
  • Manage a dedicated hospice caseload as the primary RN Case Manager
  • Perform patient assessments, admissions, and individualized care planning
  • Deliver skilled nursing care focused on comfort and dignity
  • Coordinate care with physicians and interdisciplinary team members
  • Conduct routine visits and adjust care plans as neededProvide end-of-life support, including family education and pronouncement
  • Supervise Home Health Aides in compliance with regulations
Qualifications
  • Graduate of an accredited RN program

  • Hospice or home health experience preferred
  • Strong clinical judgment and communication skills
