Job Description
The RN Case Manager coordinates the management of care to ensure optimum utilization of resources, service delivery, and compliance with external review agencies. This role will provide ongoing support through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. The RN Case Manager enhances the quality of patient management and satisfaction to promote continuity of care and cost-effectiveness by integrating case management, utilization review, and discharge planning functions.
Responsibilities
  • Conduct comprehensive assessments and care coordination for individual patient needs.
  • Implement care plans and evaluate patient progress.
  • Advocate for patients, addressing their holistic needs.
  • Ensure effective resource utilization and collaborate with the interdisciplinary team.
  • Oversee discharge planning to facilitate smooth transitions in care.
Required Experience / Certifications / Licensure
  • Minimum of 2 years of recent RN Case Management experience.
  • Experience in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) setting.
  • Hold a valid CA RN license.
  • Current BLS certification (from the American Heart Association).
