Job description

Hopscotch Primary Care is reimagining primary care for older adults in the rural communities of Western North Carolina. We give our clinicians the time, team, and tools to practice medicine the way it should be - focused on the patient, not the paperwork. That means an average of 15 patients per day, smaller panels, longer appointments with charting time built in, and a dedicated support team behind every visit.

We're hiring a Physician Assistant in Brevard, NC to deliver comprehensive, interdisciplinary primary care with a focus on patient outcomes, quality, and experience.

What You'll Do
  • Deliver comprehensive primary care to a set patient panel, including office and telehealth visits for routine and acute needs
  • Complete comprehensive annual wellness visits and health risk assessments
  • Work closely with physicians, community health workers, case managers, pharmacists, and other team members to deliver high-quality outcomes
  • Practice with a continuous-improvement mindset - following evidence-based guidelines and helping innovate care models that solve real problems for patients
  • Meet high benchmarks on quality metrics and outcomes, including patient experience, STARS/HEDIS, and utilization
About You
  • Certified Physician Assistant with an active, non-probationary North Carolina license (or eligibility to license in NC)
  • Minimum of two years' experience in a primary care specialty (internal medicine, geriatrics, or family medicine)
  • Experience with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs)
  • Proficiency with EHRs and multiple software platforms, including spreadsheets and data entry/navigation
  • Strong organizational skills and the ability to manage multiple priorities
  • US work authorization
