Job description

Title:Case Manager
Program:59th Street Women's Shelter
Salary Range: $43,558 - $47,678 Per Year
Program Description
The 59th Street Women's Shelter is a 200-bed, 24/7 emergency shelter for homeless women diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities, occupational therapy, and housing placement assistance. Additionally, a street-facing, on-site Article-28 Clinic provides primary care, and behavioral health care services.
Overall Responsibilities
Under the general direction of the Director of Social Services and the Team Leader, the Case Manager is responsible for providing required assessments, case management, and providing delivery of service to the client; these responsibilities will ultimately assist the client in addressing their dependencies and assume control of their lives to eventually live independently and return to permanent or other alternative housing in their communities. The case manager is also responsible for providing referral services to the clients on their caseload and assisting with other cases as need. The case manager will ensure that proper protocols are being followed and will ensure that chart(s) remain audit ready at all time; performs related work.
Essential Duties and Responsibilities
The essential duties of the Case Manager include but are not limited to the following activities:
  • Conducts intake and assessments, makes necessary referrals for services and completes/updates psycho-social assessments of clients
  • Provides case management services and assists clients in the creation of and adherence to their Independent Living Plans by identifying short and long-term objectives of which can be achieved
  • Inputs all client information and updates in a timely fashion into the CARES system as well as the client's physical chart
  • Works collaboratively with clients and other staff to identify and obtain financial entitlements, housing and other concrete services
  • Maintains accurate and up to date written client documentation as required and in accordance with City/State oversight agency guidelines
  • Provides guidance for clients through individualized session on issues such as achieving personal goals, finding permanent or alternative housing, maintaining sobriety, complying with medication protocols and pursuing employment opportunities
  • Maintains liaison with representatives from community organizations
  • Provides escort services to clients' appointments as needed
  • Attends and participates in the program meetings
  • Performs other duties as assigned by the Team Leader, Director of Social Services and/or Program Director
  • The Case Manager must be able to lift or move material and equipment weighing up to 15 pounds.

Qualifications:
  • A bachelor's degree in social work, public administration, psychology, education, sociology, occupational or physical therapy, or other related human services fields with a minimum of two (2) years of experience
  • An associate's degree in the aforementioned fields with three (3) years of experience
  • A High School Diploma or GED equivalent with four (4) years of experience
  • Bilingual or multilingual a plus
  • Demonstrated ability to manage and motivate staff to accomplish stated program goals and objectives while developing their individual and group skills.
  • Ability to develop and foster, through demonstration, a team spirit, enabling staff to overcome the challenges of providing needed services to a special client population.
  • Excellent oral, writing, and listening skills are a must.
  • The ability to work well within a highly pressurized, deadline environment, while meeting the short-term and long-term mandates of the program.
  • An ability to interface with clients as well as all levels of staff.
  • Knowledge of case management software as well as proficiency in Microsoft Office Suite
