QLA Case Manager / Care Coordinator Level 2
Location: Wyoming County and McDowell County, WV
Position Type: Full-Time

Position Summary

The QLA Case Manager/Care Coordinator Level 2 assists individuals in accessing essential behavioral health, medical, social, educational, and community resources. This position provides care coordination and support services to help clients overcome barriers, improve stability, and achieve recovery goals.

The successful candidate will work collaboratively with community partners, healthcare providers, recovery organizations, and internal treatment teams to ensure individuals receive comprehensive, person-centered care.

Essential Responsibilities
  • Coordinate behavioral health, medical, social, educational, and community-based services for clients.
  • Conduct assessments and identify client needs and barriers to care.
  • Develop, implement, and monitor individualized service plans.
  • Connect clients with treatment providers, recovery resources, housing assistance, transportation, and other supportive services.
  • Maintain accurate and timely documentation in accordance with organizational, state, and federal requirements.
  • Collaborate with multidisciplinary treatment teams and community partners.
  • Participate in outreach efforts and provide support to individuals impacted by substance use and mental health challenges.
  • Assist clients in navigating healthcare systems and accessing appropriate resources.
  • Promote recovery-oriented and trauma-informed approaches to service delivery.
Education & Qualifications

Applicants must meet one of the following qualifications:

  • Bachelor's or Master's degree in a human services-related field (in accordance with West Virginia Board of Social Work requirements); OR
  • Licensed Psychologist (Master's or Doctoral degree from an accredited program); OR
  • Licensed Registered Nurse.
Preferred Qualifications
  • Experience working in behavioral health, substance use treatment, recovery services, case management, or care coordination.
