Job description

Job Title: Case Manager Location: Los Angeles, CA Type: Contract Compensation: $23 hourly Contractor Work Model: Onsite Time-Limited Subsidy (TLS) Case Manager The TLS Case Manager provides housing-focused case management to individuals and families experiencing homelessness. This role supports rapid rehousing efforts, helps clients secure and maintain permanent housing, and ensures compliance with program and funding requirements. Key Responsibilities:
  • Manage a caseload and provide strengths-based, housing-focused case management
  • Develop housing retention plans and conduct regular client follow-ups
  • Assist clients with housing applications, landlord coordination, and rent sustainability planning
  • Maintain accurate documentation in HMIS and ensure timely reporting
  • Verify eligibility and ensure compliance with program standards
  • Coordinate services with internal teams and community partners
  • Conduct field/home visits (including some evenings as needed)
Qualifications:
  • Associate degree in social services or related field OR 2+ years of case management experience
  • Experience working with homeless populations and handling case loads preferred
  • Knowledge of rapid rehousing, Housing First, and trauma-informed care
  • Proficient in Microsoft Office; HMIS experience preferred
  • Strong communication and documentation skills
