JOB DESCRIPTION - DIRECT CARE WORKER Employee Name:

REPORTS TO: Service Plan Manager/Program Director
FUNCTION: This position has the responsibility of being the primary staff for clients. As such the direct care staff person is responsible for providing the prescribed program of Active Treatment to each client based on the Individual Plan of Care. This employee provides the actual training, and also provides the day to day supervision, guidance, and support to clients, which may be required to facilitate each resident's individual learning and development. This employee works under the direction of the service plan manager or the assistant service plan manager as designated.
JOB REQUIREMENTS: This position requires a high school diploma or a GED certificate. Additionally persons must be at least 23 years of age. An exception may be made by the agency director if a person has at least five (5) years of experience in a residential care facility, but no person shall be employed who is less than twenty three (23) years of age.
An employee who fills this position must participate in all trainings, must work when assigned to work by the service plan manager, and must follow the rules and regulation of HCI . Additionally, staff persons must follow all directions, procedures and guidelines for the provision of individual programs of service to each resident.
PRINCIPLE ACTIVITIES:

โ™ขWhere applicable, implement according to requirements individual clients or group Behavior Management Plan.
โ™ขWhere applicable, supervise the self administration of medication by residents.
โ™ขKnow Direct Care Worker Person Checklist (see handbook). Assist in developing and implementing Active Treatment.
โ™ขParticipate in intake and individualized plan of care and development of same.
โ™ขProvide individual and group counseling.
โ™ขRecord all accident/incidents occurring on each shift.
โ™ขInspect living areas for repairs and cleanliness.

โ™ขBe thoroughly familiar with all policies and procedures relative to the resident's (discipline, medical emergency, safety, and runaways).
โ™ขSupervise activities, events and functions.
โ™ขProvide necessary transportation.
โ™ขRestrict inappropriate behaviors in residents and assist them in developing alternate appropriate behavior.
โ™ขTrain residents in good personal habits, i.e., grooming, eating, hygiene.
โ™ขSupervise, participate in, and train residents in cooking, meal planning, shopping, and all household maintenance activities.
โ™ขSupervises, guides, trains, participates in, and encourages participation in a variety of leisure time/recreational and social skills, and development activities.
โ™ขOther duties as assigned by service manager, assistant service plan manager or his/her designee.
โ™ขThis is a full time position.
