We are currently seeking a Care Plan Coordinator.

Job Responsibilities and Duties:
  • Conduct comprehensive assessments and evaluations of patient care needs, ensuring compliance with guidelines.
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams.
  • Perform utilization reviews to ensure appropriate resource allocation and adherence to Medicare regulations.
  • Maintain accurate medical documentation and records in EHR systems, ensuring HIPAA compliance.
  • Provide guidance and support to nursing staff regarding patient care and documentation practices.
Requirements:
  • Unencumbered RN or LPN license
  • 2–5 years of experience in long-term care
  • Knowledge of Medicare regulations
  • Commitment to Total Quality Management
  • Working knowledge of Case Mix Index, Quality Measures tracking, QRP, and Value Based Purchasing
