Description:
POSITION PURPOSE
  • Registered Nurse, Nurse Coordinator for Michigan Heart.
  • Contingent, Non-Benefited position offering flexible scheduling. Scheduled hours are weekdays, 7:30 a.m. to 4 p.m. (No Weekends, No Holidays, No On-Call requirements).
  • Michigan Heart is seeking a Registered Nurse Coordinator to provide rounding and care coordination support for hospitalized cardiology patients. The position serves as a key liaison between providers, patients, families, and care teams to facilitate timely, high-quality care.
  • Michigan Heart, based at Trinity Health Ann Arbor, is a nationally recognized cardiovascular program providing comprehensive heart and vascular care through a team of physicians, advanced practice providers, nurses, and specialized clinicians.

MINIUMUM QUALIFICATIONS
  • Current Michigan Nursing License
  • Graduation from accredited nursing program: BSN Required
  • Minimum of three years' staff experience. One-year related practice experience in clinical area.

WHAT YOU WILL DO
  • Coordinates care for hospitalized cardiology patients in collaboration with providers and interdisciplinary care teams.
  • Conducts daily patient rounding to identify care needs, barriers, and opportunities to improve outcomes.
  • Identifies and monitors high-risk cardiac patients, coordinating services and follow-up needs throughout the hospital stay.
  • Serves as a resource for patients and families, providing education, support, and guidance throughout the care journey.
  • Coordinates testing, procedures, consultations, and follow-up services to support timely and efficient care.
  • Facilitates communication among patients, providers, caregivers, and care teams to ensure continuity of care.
  • Identifies and helps resolve barriers to care, including financial, transportation, language, and social needs.
  • Participates in quality improvement, workflow optimization, and program initiatives to enhance patient outcomes and operational effectiveness.
  • Serves as a clinical resource and mentor, supporting evidence-based practice and department initiatives.
