  • 7pm - 7am - Friday / Saturday
  • 7pm - 7am - PRN - Various Nights & Weekend Nights
Pay:
  • LPN - $48 / hour

Join the world of HOME HEALTH CARE, where you can provide ONE-ON-ONE care to patients in the comfort of their home. You will work with ONE PATIENT for your complete shift. You will get to know your patients and their families and learn about the needs they may have in their home. Then you will plan, organize, and assist with home health care service with your focus on just one patient for your shift.
As a Skilled Nurse (Registered Nurse or License Practical Nurse), you will assist the patient with daily living activities, ensure their comfort, and support their overall well-being to enhance their quality of life.

Responsibilities may include the following:
  • Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
  • Demonstrate understanding organizational and nursing policies and procedures.
  • Demonstrate strong communication skills, both spoken and written.
  • Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
  • Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
  • Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
