Position Overview

Mind Canvas Counselors is actively seeking a dedicated and passionate Licensed Clinical Social Worker (LCSW) to join our innovative team. This full-time remote position offers the unique opportunity to provide critical support to individuals and families in need, particularly those receiving care in a Hospice Center environment. You will play an essential role in delivering compassionate, high-quality social services while working closely with clients to address their emotional, social, and practical needs.

Work Environment

This role allows for remote work, providing flexibility and the ability to maintain a work-life balance while still engaging deeply with clients. You will connect with clients via virtual platforms and phone consultations, identifying their unique needs and challenges from the comfort of your home or chosen workspace. Although your primary duties will be performed remotely, occasional visits to the Hospice Center may be necessary to enhance client engagement and care coordination, depending on client needs.

Key Responsibilities
  • Conduct comprehensive assessments to identify clients' social, emotional, and environmental challenges, ensuring a holistic understanding of their situation.
  • Develop and implement personalized service plans that outline specific goals, interventions, and methods for progress monitoring, tailoring each plan to individual client needs.
  • Deliver effective counseling services to individuals and families facing various life challenges, using evidence-based practices to foster positive outcomes.
  • Provide crisis intervention, conducting risk assessments and developing safety plans as necessary to ensure client well-being.
  • Engage in meaningful collaboration with community agencies to coordinate services, advocate for clients, and streamline their access to essential resources.
  • Participate in community outreach activities aimed at increasing awareness of mental health issues and available agency services.
  • Contribute to team efforts by attending case conferences, staff training, and team meetings to share insights and improve service delivery.
Patient Population / Client Focus

In this role, you will primarily work with clients who are experiencing significant life transitions related to terminal illnesses and end-of-life considerations. Your sensitivity to the unique emotional needs associated with hospice care will be crucial as you assist clients and their families in navigating grief and loss, providing empathetic support, and connecting them with necessary community resources throughout their journey.

Qualifications
  • A recognized Master’s degree in Social Work from an accredited institution.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Strong understanding of social and economic justice issues, along with familiarity with community resources and trauma-informed care practices.
  • Demonstrable excellence in communication, cultural competence, and problem-solving abilities that facilitate positive client relationships and outcomes.
Preferred Experience
  • Prior experience in a social service agency, particularly within hospice or end-of-life care environments, is highly desirable.
  • Experience working with diverse populations and understanding the unique challenges they face, especially in crisis situations.
Compensation and Benefits
