General Purpose

The Physical Therapist evaluates residents, develops individualized treatment plans, and provides therapeutic interventions designed to improve mobility, strength, balance, endurance, and functional independence.

Essential Duties
  • Evaluate residents and determine physical therapy needs and rehabilitation potential
  • Develop, implement, and modify individualized treatment plans
  • Provide treatment focused on gait training, transfers, balance, strengthening, mobility, and pain management
  • Communicate with physicians, nursing staff, residents, and families regarding progress and goals
  • Participate in care plan meetings and interdisciplinary team conferences
  • Complete timely, accurate, and compliant documentation
  • Educate residents and caregivers on exercises, mobility safety, and fall prevention
  • Recommend assistive devices and durable medical equipment such as walkers, canes, and wheelchairs
  • Supervise PTAs and therapy aides as appropriate
  • Maintain a safe treatment environment and follow infection control procedures
Qualifications

Education:

  • Doctorate or Master's Degree in Physical Therapy from an accredited program required

License:

  • Current California Physical Therapist license in good standing

Preferred Skills:

  • Experience with Casamba and PointClickCare
  • Strong communication and interpersonal skills
  • Excellent clinical judgment and problem-solving abilities
  • Passion for patient-centered care and functional outcomes
